Pre-Participation Physical Form - View HERE. Physicals are valid for two years from the date signed by the physician. If you do not already have a completed physical form signed by a physician, you will be required to upload this completed form in the registration process. If your physical form is still valid for this school year, please see Alternate Year Athletic Permit Card below.

Alternate Year Athletic Permit Card - View HERE. A new permit card is required for every school year. You will be required to upload this card during the registration process.
